In consideration of PACE Staffing Network's evaluation of my application and placement counsel:

I certify that the information I have provided PACE Staffing Network is true and complete to the best of my knowledge. I understand that deliberately providing false information during the application process forfeits my eligibility to be represented by PACE Staffing Network, and I may be removed from my assignment and/or terminated for cause from the employment of PACE Staffing Network.

I hereby authorize PACE Staffing Network to obtain information from previous employers, former work associates, schools, licensing authorities, and/or law enforcement agencies concerning my eligibility for employment.

I hereby provide PACE Staffing Network authorization to disclose any information they obtain as part of their evaluation of my eligibility for employment with their clients for the purposes of placing me on a work assignment or referring me for hire. I further release any person or entity from all liability related to issuing information about my qualifications for employment.

I understand that PACE Staffing Network may request that I take a drug test if required by the employment or work practices of their client to whom I would otherwise be eligible for referral. I further understand that PACE may disclose the results of such test to the client.

I understand that I have the right to review any written report that becomes part of my employment file as a result of PACE's investigation of my eligibility for employment.

In consideration of my employment by PACE Staffing Network:

After Accepting an assignment, I cannot leave that assignment to accept another assignment at the same company, through another staffing company, without the written permission of PACE Staffing Network. Violation of this agreement may subject me to penalties and fees for damages incurred to PACE Staffing Network because of my actions.

I release PACE Staffing Network from any liability for any descriptions of my work performance it may provide to a future employer.

I agree that all information and records I produce or come in contact with while on assignment is the sole property of that client and will be considered proprietary and confidential. I agree not to use or disclose any information or work product I obtain or create while on assignment to any third party or to utilize said information or property for my own benefit.

I agree that my employment can be terminated with or without cause at the option of either PACE Staffing Network or myself.

I agree that if at any time I make a claim for a worker's compensation benefit based on an injury that occurs while on assignment via PACE Staffing Network, I will submit to any and all requests from PACE Staffing Network to an examination by a physician selected by them.
